WEEKDAY Shule Registration

  • REGISTRATION TO ATTEND WEEKDAY SYNAGOGUE SERVICE

  • IMPORTANT NOTE:

    • A separate Registration Form must be completed for EACH PERSON who wishes to attend each service.
    • Registration Forms for family or other groups will not be considered.
    • A Registration Form will be required to be completed and submitted for each person and for each service that person wishes to attend.
    • The total number of persons who may attend each service is strictly limited by government regulations, Sydney Beth Din's guidelines and Dover Heights Shule Inc's ("DHS" or "Shule") policy.
    • Completing and submitting Registration Form does NOT guarantee that you will be permitted to attend the relevant service.
    • FOR SHABBAT AND YOM TOV SERVICES ( CLICK HERE to access the Shabbos Registration Form) - You may attend the service ONLY if you receive formal notification from DHS beforehand via email confirming that you have been accepted, and you comply with the DHS Service policy. Otherwise, you will not be allowed to enter the Shule's premises or attend the Shule's service.
    • FOR REGULAR WEEKDAY SERVICES ONLY- If you have not received formal notification from DHS beforehand via email confirming that you have been accepted to attend for the relevant weekday service, you may still attend the weekday service provided:

    1.    the total number of persons who may attend that service has not been reached;

    2.    you complete and submit a Registration Form to the CovidSafe Warden's satisfaction; and

    3.    comply with the DHS Service policy.

    Otherwise, you will not be allowed to enter the Shule's premises or attend the Shule's service.

    • Admission to each service will be strictly monitored and controlled by a designated DHS CovidSafe Warden ("CovidSafe Warden") and any other designated personnel and security officers.
    • These procedures are put in place to protect the Shule community– Please respect the designated Shule Wardens who are trying to ensure compliance.

    By submitting this Registration Form to DHS, I am agreeing to each of the statements below.

  • On arrival at DHS I will provide my correct name and may be asked to produce photo ID (eg driver’s license). I understand and agree that if I fail to answer any questions from the CovidSafe Warden or other DHS designated personnel I may not be allowed to enter the Shule's premises or attend Shule's service.

    Throughout the time of my attendance at the Shule, except when entering or leaving, I will remain in one demarcated seat, unless called to the Bimah,  as reasonably directed by the CovideSafe Warden or to go to the washroom.

    In particular, throughout the time of my attendance at the Shule, I will NOT engage in hand-shaking, hugging, kissing or moving around the Shule to greet people or to socialise nor will I kiss the Torah.

    Throughout the time of my attendance at the Shule, and when entering and leaving, I will immediately and fully comply with all DHS policies (including CovidSafe Plan) and the directions of the CovidSafe Warden or other DHS designated personnel. My failure to comply with any such instructions or to comply with DHS' CovidPlan and policies may result in me being denied entry to the Shule or being asked to leave the Shule's premises.

    Throughout the time of my attendance at the Shule, and when entering and leaving, I will practise social distancing by strictly observing the rule of no more than one person per 4 square metres, and maintaining a physical distance of at least 1.5 metres from all other persons, including family members and friends. This will also apply if I am called to the Bimah.

    Upon leaving the Shule I will not linger, or join in gathering with any other people, in the Shule's premises or its immediate precincts.

    I consent to the Shule and its staff collecting, storing, using and disclosing my personal information (including my health information) solely for the purposes of assessing this application, and disclosing only my name, address and contact details to any government health official on request so that I can be contacted promptly if it should transpire that someone who has tested positive to the COVID-19 virus attended the same service.

    I am the applicant, and I have personally completed all items in this application form.

    I acknowledge and agree that the information I have provided is true and correct to the best of my knowledge, and that I have not knowingly provided information, which is false, misleading or incomplete.

    I understand that the Shule, its office-bearers, clergy, staff and contractors, and other persons attending the service, will rely on the truth and accuracy of the information I have provided in this application in seeking to mitigate the risk of COVID-19 infection by community spread within the Shule and its precincts, and the risk of severe or lethal consequences if infection occurs.

    If, between the time of submitting this application to the Shule and the scheduled time of commencement of the service I wish to attend, my circumstances change in any way that would result in any information I have provided no longer being true, correct and complete, I will immediately notify the Rabbi, the President of the Shule or a staff member at the Shule office of full details of the changed circumstances. I will not attend if there is an increase in my COVID19 risk factors as detailed above.
